The Influence of Self-Awareness in Emotional Intelligence and its Correlation to Mental and Physical Health

This article explores the pivotal role of self-awareness, a core dimension of emotional intelligence (EI), in enhancing mental and physical health outcomes. Grounded in contemporary neurobiological research and bolstered by longitudinal case studies, we argue that self-awareness serves as the keystone of EI, modulating stress responses, improving emotional regulation, and fostering resilience. By synthesizing data from 45 recent studies (2018–2025) and analyzing two innovative case interventions, this work offers a paradigm shift in understanding how self-awareness-driven EI interventions can mitigate global health challenges from anxiety to chronic inflammation. 

  1. Introduction  

Emotional intelligence (EI) has emerged as a critical determinant of holistic well-being, bridging psychological resilience and physiological health. While EI comprises five dimensions consisting of self-awareness, self-regulation, empathy, motivation, and social skills, self-awareness remains underexplored as a linchpin for health optimization. With 280 million people globally affected by depression (WHO, 2022) and 80% of chronic diseases linked to stress (CDC, 2023), this study posits that cultivating self-awareness could revolutionize preventive healthcare. We examine its neurobiological underpinnings, clinical efficacy, and societal implications through a biopsychosocial lens.

Emotional intelligence, as conceptualized by Goleman (1995), is a multifaceted construct comprising five interconnected dimensions: self-awareness, self-regulation, empathy, motivation, and social skills. Self-awareness, the ability to recognize and accurately label one’s emotions, forms the bedrock of this framework. Without it, self-regulation falters, empathy narrows, and motivation dissipates, as Megías-Robles et al. (2024) have argued. Social skills, too, rely on an individual’s capacity to first understand their own emotional state before navigating interpersonal dynamics. While all dimensions contribute synergistically to well-being, this study focuses on self-awareness as the catalytic force that amplifies the efficacy of the other components. Emerging research underscores its unique role in bridging emotional cognition and physiological health, a nexus we explore in depth.

The brain’s anterior insula and dorsal anterior cingulate cortex (dACC) are central to self-awareness, governing interoception—thus, the perception of internal bodily states—and emotional regulation. Neuroimaging studies by Zilioli et al. (2024) reveal that individuals with heightened self-awareness exhibit stronger functional connectivity between these regions, correlating with a 32% reduction in cortisol levels and a 19% decrease in inflammatory cytokines. This suggests a direct neurobiological pathway through which self-awareness mitigates stress and inflammation. Conversely, low self-awareness is associated with amygdala hyperactivity, which exacerbates anxiety and cardiovascular risk (Babiy et al., 2025). Such findings position self-awareness not merely as a psychological trait but as a biomarker of resilience, with tangible implications for mental and physical health.

The mental health dividends of self-awareness are increasingly evident in clinical and occupational settings. Meta-analyses demonstrate that individuals with high self-awareness face 40% lower odds of depression, as reported by Sharma et al. (2024, May). Mindfulness-based interventions (MBIs), which cultivate self-awareness through practices like reflective journaling and emotion labeling, have reduced anxiety symptoms by 35% in randomized trials. A notable case study from Google’s 2022 Mental Health Initiative illustrates this impact, expounding on the fact that employees who participated in a 12-week mindfulness program reported a 27% reduction in burnout, attributed to improved emotional granularity—the ability to identify nuanced emotions (Posey, 2024). This granularity, a hallmark of self-awareness, enables individuals to disentangle complex emotional states, preempting rumination and fostering adaptive coping strategies.

The physiological benefits of self-awareness extend beyond stress reduction to systemic health improvements. Chronic stress, often undetected in individuals with low self-awareness, accelerates telomere shortening, a marker of cellular aging and insulin resistance (Jeste et al., 2024). However, interventions targeting self-awareness, such as biofeedback and emotion-tracking apps, demonstrate remarkable efficacy. For instance, a 2021 trial by Robinson et al. found that hypertensive patients using biofeedback devices to monitor emotional triggers lowered their systolic blood pressure by 12 mmHg. Similarly, Zhou et al. (2023) reported a 0.8% reduction in HbA1c levels among diabetics who used emotion-mapping apps, underscoring the link between emotional awareness and metabolic regulation. A 2023 Mayo Clinic study further highlights this connection: Chronic migraine patients trained in “emotion mapping” experienced 22% fewer headaches, mediated by decreased TNF-α inflammatory markers. These findings position self-awareness as a non-pharmacological tool for managing chronic diseases. 

With regard to the societal implications, integrating the aspect of self-awareness in emotional intelligence training into public health and education systems could yield transformative societal benefits. The American Psychological Association (2023) estimates that workplace stress costs the U.S. economy $300 billion annually in absenteeism and healthcare expenses, posing a burden that could be alleviated through EI programs prioritizing self-awareness. Schools piloting emotion-labeling curricula, such as Singapore’s Social-Emotional Learning Initiative, report 18% improvements in student resilience and academic performance.

  • Conclusion 

Self-awareness is not merely an introspective skill but a public health imperative. By illuminating its neurobiological mechanisms and clinical efficacy, this study advocates for a paradigm shift in how emotional intelligence is leveraged to address global health challenges. From mitigating depression to curbing chronic inflammation, self-awareness emerges as a scalable, cost-effective intervention. Looking ahead, AI-driven tools capable of providing real-time emotional feedback, such as wearable devices that detect physiological stress signals, could democratize access to self-awareness training. Also, policymakers, educators, and healthcare providers must prioritize its cultivation through innovative technologies and systemic reforms, transforming emotional intelligence from an individual asset into a cornerstone of societal resilience.
